Additional Interpretation: The T cell receptor gene undergoes programmed genetic rearrangement in the V-D-J region during the maturation of all T lymphocytes. The rearrangement process results in V-D-J regions that are unique in size and sequence for each T lymphocyte. Gene regions flanking the rearranged areas are conserved. Therefore, when the unique, rearranged sequences are amplified using primers that bind to the conserved flanking regions, a clonal population of cells yields prominent amplified products within an expected size range.

Methodology: Testing for the presence of a rearrangement of the T-cell receptor gamma and beta genes was performed by PCR using fluorescently labeled primers to multiple V and J exon regions within the T-gamma gene and V, D and J exon regions within the T-beta gene combined with capillary electrophoresis of the amplified products (Invivoscribe®). The specimen is judged to be undetectable if no prominent band is detected in the 55-85, 155-185, 200-235 or 235-270 bp ranges for T-gamma gene and 170-210 or 240-325 bp ranges for T-beta gene. The presence of amplifiable DNA in the extracted specimen is verified by amplification of the DNA using primers to several human genes.
