Additional Interpretation: The immunoglobulin heavy and light chain genes undergo programmed genetic rearrangement in the V-D-J region during the maturation of all B lymphocytes. The rearrangement process results in V-D-J regions that are unique in size and sequence for each B lymphocyte. Gene regions flanking the rearranged areas are conserved. Therefore, when the unique, rearranged sequences are amplified using primers that bind to the conserved flanking regions, a clonal population of cells yields prominent amplified products within an expected size range.

Methodology: Testing for the presence of a rearrangement of the immunoglobulin heavy chain (IgH) gene is performed by PCR using fluorescently labeled primers to the FR1, FR2 and FR3 regions combined with capillary electrophoresis of the amplified product. The specimen is judged to be undetectable if no predominant band is detected in the 70-360 bp range (Invivoscribe®). The presence of amplifiable DNA in the extracted specimen is verified by amplification of the DNA using primers to human HLA-class II.
